What is Foundayo, and how does it work?

Foundayo (orforglipron) is a tablet made by Eli Lilly, whose active ingredient is orforglipron. FDA approved for chronic weight management. A GLP-1 receptor agonist that works as a tablet rather than an injection, activating the same appetite-regulating receptor. In its pivotal trial it produced 11.1% mean weight loss. It costs $299 a month once titrated, read from its own published page on 2026-09-12 — the maintenance rate rather than the starting dose, because the starting dose is not the dose anyone stays on.

How does Foundayo work? The label's words

FOUNDAYO is a GLP-1 receptor agonist that binds to and activates the human GLP-1 receptor. GLP-1 is a physiological regulator of appetite and caloric intake. GLP-1 receptors are present in brain regions that regulate appetite. In animal studies, orforglipron distributed to and activated neurons in brain regions that regulate appetite and food intake.

What is Foundayo approved for?

  • Chronic weight management

    To reduce excess body weight and maintain weight reduction long term in adults with obesity, or adults with overweight in the presence of at least one weight-related comorbid condition.

Approvals are quoted from the label, because the wording is what a prior authorization is judged against. Which approval a plan cannot exclude — a weight-loss exclusion is written against the use, not the molecule.

How much weight did Foundayo lose in its trial?

11.1% of body weight. ATTAIN-1, Trial 1 on the label, 72 weeks: 11.1% mean weight loss at 17.2 mg against 2.1% on placebo, intention-to-treat.

It carries a boxed warning — the strongest the FDA issues

The label carries the GLP-1 class boxed warning for thyroid C-cell tumors, and says why it applies here: GLP-1 receptor agonists that are pharmacologically active in rats and mice caused thyroid C-cell tumors at clinically relevant exposures. Orforglipron is not pharmacologically active in rats or mice and did not produce tumors in rodents; because it is active at the human GLP-1 receptor, the label says the human relevance of those rodent tumors has not been determined. It is contraindicated outright with a personal or family history of medullary thyroid carcinoma or MEN 2.

How much does Foundayo cost?

Is Foundayo insulin?
No. Foundayo is a GLP-1 receptor agonist, not insulin: it acts on the body's own insulin release rather than replacing insulin. That is why these drugs' labels warn of low blood sugar mainly when they are taken with insulin or a sulfonylurea.
