Orforglipron is not pharmacologically active in rats or mice and did not produce tumors in rodents; because it is active at the human GLP-1 receptor, the label says the human relevance of those rodent tumors has not been determined. It is contraindicated outright with a personal or family history of medullary thyroid carcinoma or MEN 2.

Who it is for: Part D beneficiaries 18 or older whose prescriber submits a prior authorization attesting the drug treats obesity, and who have a BMI of 35 or higher, a BMI of 30 or higher with diastolic heart failure, uncontrolled hypertension or stage 3a chronic kidney disease, or a BMI of 27 or higher with prediabetes, a previous heart attack or stroke, or symptomatic peripheral artery disease.
