Does Mounjaro cause constipation?
Yes, and it is on the label rather than only in reports: constipation appears among the common adverse reactions in Mounjaro's FDA prescribing information, published 2026-09-02. Listed means it was seen often enough in the trials to be printed, not that it happens to everyone. The same reaction is on the label of Zepbound too. Tirzepatide is the same molecule in all of them, so this belongs to the substance rather than the brand, and switching between them is no way out of it. The same slowed gut transit that produces the fullness produces constipation at the other end. Eating much less compounds it: less food means less fiber and less fluid, and both are what keep things moving. It clusters in the days after each dose increase. Mounjaro steps up on a published schedule. Most people see a few rough days after each step and a settling in between, not one long stretch. Once you are at 2.5 mg to 15 mg once weekly, set by blood sugar control and no longer stepping up, the trigger stops recurring. What helps first: fluid first. It is the most common deficit and the easiest to fix. It stops being ordinary at a specific line: no bowel movement for several days with abdominal pain, distension or vomiting. That combination needs assessment rather than another laxative.
How long does Mounjaro constipation last?
One injection a week. Each row is a step up, and the days after each one are when this tends to arrive.
| Weeks | Dose |
|---|---|
| 1–4 | 2.5 mg |
| 5 and on | 2.5 mg → 15 mg |
What helps with constipation on Mounjaro?
- 1Fluid first. It is the most common deficit and the easiest to fix.
- 2Fiber added gradually rather than all at once, which otherwise trades constipation for bloating.
- 3Movement, which does more for gut transit than most supplements.
- 4Asking the prescriber about a stool softener or an osmotic laxative before reaching for a stimulant one.
Same molecule and the same pen strengths as Zepbound, on the diabetes label, but a different schedule. Mounjaro goes up from 2.5 mg only if glucose control needs it, in 2.5 mg steps at least 4 weeks apart, and stops at 10 mg for children 10 and older. Gastrointestinal reactions made 3.0% of adults stop at 5 mg, 5.4% at 10 mg and 6.6% at 15 mg, against 0.4% on placebo, and most nausea, vomiting and diarrhea comes during dose escalation and eases over time. Side effects that do not settle are a conversation with the prescriber rather than a reason to quit unannounced.
When is constipation on Mounjaro serious?
No bowel movement for several days with abdominal pain, distension or vomiting. That combination needs assessment rather than another laxative.
Separately, Mounjaro carries a boxed warning. In rodents this molecule caused thyroid C-cell tumors at human-relevant doses. Whether it does so in people is unknown, and the label carries that as a boxed warning. It is contraindicated outright with a personal or family history of medullary thyroid carcinoma or MEN 2.
Constipation on Mounjaro, Zepbound — the same molecule, on every one of their labels
Tirzepatide is the active substance in Mounjaro and Zepbound, and every one of their FDA labels lists constipation among the common adverse reactions. That makes it a property of the molecule rather than of the brand: moving between these products is a change of approved indication and dose ceiling, not a way out of this.

Would switching drugs help?
Constipation is on 12 of the 13 FDA labels tracked here, and every other product on the same molecule lists it too, so a switch within the molecule is not a way out of it. What a switch does change is what you pay, and Mounjaro is $499 a month at the maintenance dose here.
