    Mochi's compounding supplier was Aequita Pharmacy of Kirkland, Washington, shipping 20,000 prescriptions a week at its peak. Washington Department of Health regulators substantiated that unlicensed temporary workers, recruited from home-improvement store car parks, handled prescriptions inside the facility, and found the pharmacy was letting pharmacy assistants compound in the clean room without having completed the necessary training. Inspectors said they discovered GLP-1 prescriptions ready to mail that had been expired for nearly nine months, at least three of them. Witnesses reported glass particles in some compounded product after a switch to cheaper vials that shattered; whether any reached patients is not established. Staff alleged that when ice packs ran out, Mochi's chief executive suggested frozen Otter Pop popsicles instead, and patients wrote online about them leaking over the medication. Visiting a warehouse unannounced in March 2025, inspectors opened a refrigerator at a site the manager said held only packaging and found bags of peptide base powders. The department issued Aequita a Notice of Immediate Jeopardy, stating that its deficient practices had placed the health and safety of individuals at risk of serious injury, serious impairment or death.

The complaint against Mochi alleges it moved patients onto compounded tirzepatide with different added ingredients, including niacinamide, changing the formulation at least five times in eight months, and that some sellers offered forms never studied at all, such as oral tablets and drops. Against Mochi and Fella, Lilly also alleged the unlicensed practice of medicine.
