It carries a boxed warning — the strongest the FDA issues
In rodents this molecule caused thyroid C-cell tumors at human-relevant doses. Whether it does so in people is unknown, and the label carries that as a boxed warning. It is contraindicated outright with a personal or family history of medullary thyroid carcinoma or MEN 2.

NovoCare self-pay, read 19 Aug 2026: patients new to the savings offer pay $199 for the first two monthly fills of 0.25 mg and 0.5 mg, through 31 Dec 2026. After that $349 for 0.25–2.4 mg and $399 for Wegovy HD 7.2 mg. The page reads "NovoCare® Pharmacy pay $199 for each month of 0.25 mg & 0.5 mg" and "then $349 per month for Wegovy® 0.25 mg, 0.5 mg, 1 mg, 1.7 mg, or 2.4 mg". One month is one box of four weekly pens, so a year is 13 fills.
What this page used to say
- 2026-09-13 — Wegovy's weight at the start of the withdrawal trial, and the loss before randomization. Was: 106.8 kg at the start, the mean of all 902 enrolled, against 96.1 kg at randomization for the 803 randomized: a run-in loss of 10.7 kg, or 10% of body weight. Now: 107.2 kg at the start and 96.1 kg at randomization, both for the 803 randomized: a run-in loss of 11.1 kg, or 10.4% of body weight.
- 2026-09-13 — How many Wegovy run-in dropouts stopped because of side effects. Was: Ninety-nine of the 902 enrolled stopped during the 20-week run-in, most of them because of adverse reactions. Now: Ninety-nine of the 902 enrolled stopped during the 20-week run-in; adverse reactions were the most common reason, for 48 of them.
- 2026-09-12 — Why the label allows 1.7 mg of Wegovy. Was: The recommended maintenance dose; 1.7 mg is the alternative if 2.4 is not tolerated. Now: 1.7 mg is a maintenance dose in its own right on the label, chosen on response as well as tolerability.
- 2026-09-11 — A point in its favor that no longer stood. Was: New patients pay $199 for the first two fills through 31 December 2026, which covers most of the titration ramp. Now: New patients pay $199 for the first two fills through 31 December 2026 — the first two of the four titration months.
- 2026-09-11 — A drawback that no longer stood. Was: About 14.9% mean loss in STEP 1 against roughly 20.9% for tirzepatide — meaningfully weaker. Now: Weaker than tirzepatide where the two were tested head to head: in SURMOUNT-5, weight fell 13.7% on semaglutide against 20.2% on tirzepatide over 72 weeks.
- 2026-09-11 — What the dosage page said the steps cost. Was: Four months of titration at $199 is the cheap part; month five onwards is the price you live with. Now: The $199 covers the first two fills only; from the third month the price is the $349 you live with.
- 2026-09-05 — Said that the MASH indication is an accelerated approval. Was: FDA approved for cardiovascular risk reduction, chronic weight management, and MASH with moderate to advanced fibrosis. Now: The same three, with MASH marked as accelerated approval and the fibrosis stages named.
- 2026-09-05 — Rebuilt the serious reactions as the label's section 5, in the label's order. Was: Five items: acute pancreatitis, gallbladder disease, acute kidney injury, diabetic retinopathy complications, pulmonary aspiration. Now: Nine, in label order: acute pancreatitis (5.2), acute gallbladder disease (5.3), hypoglycemia (5.4), acute kidney injury (5.5), severe gastrointestinal reactions (5.6), hypersensitivity reactions (5.7), diabetic retinopathy complications (5.8), heart rate increase (5.9), pulmonary aspiration under anesthesia (5.10).
- 2026-09-05 — Removed the suicidality warning and added dysesthesia and hair loss to the common reactions on the side-effects page. Was: Serious reactions included 'Suicidal behavior and ideation — the label says to monitor for it'. The common list had 15 items and omitted dysesthesia and hair loss. Now: The suicidality line is gone. The common list is the label's own 17-item list in the label's order, including dysesthesia (altered skin sensation, mostly at 7.2 mg) and hair loss.
- 2026-09-05 — Added the label warning against sharing a pen between patients. Was: The serious reactions list did not mention pen sharing. Now: Never share a FlexTouch pen between patients, even with a new needle (5.11) — sharing risks passing on blood-borne infection.
- 2026-09-05 — Added the 7.2 mg (Wegovy HD) step, the missed-dose restart rule, and corrected the maximum dose on the dosage page. Was: Maximum dose: 2.4 mg once weekly. The ladder ended at 2.4 mg, the page said higher doses are not more effective, and the missed-dose answer covered a single missed dose only. Now: Maximum dose: 7.2 mg once weekly (Wegovy HD) for weight reduction in adults; 2.4 mg for cardiovascular risk reduction, for ages 12 to 17 and for MASH. The ladder gains an optional 7.2 mg step from week 21 at the earliest. Missed dose: take it if the next one is more than 2 days away, otherwise skip; after 2 or more consecutive missed doses, restart escalation at a lower dose. The sentence about higher doses is gone.
- 2026-08-19 — Self-pay pricing, listed as a flat figure. Was: $349 flat for every standard dose, listed as a point in its favor. Now: $199 for the first two fills for new patients through 31 Dec 2026, then $349 for 0.25–2.4 mg and $399 for Wegovy HD 7.2 mg.
How much does Wegovy (semaglutide injection) cost a month?
halfrx records Wegovy (semaglutide injection) at $199–$399 a month all-in, read from its own published page on 2026-09-22. The figure that matters is $349 once titrated, because the starting dose is not the dose anyone stays on, and that is the number this site scores. It carries one flag on this site: boxed warning. A pharmacy acquires the same drug for about $1,306 a month under the federal NADAC survey, which is the comparison the price above is worth reading beside. It scores 6.9 out of 10 on a published formula that is arithmetic over the facts on this page, not an editor's opinion. The cheapest verified maintenance price on the site is $75, so this sits above the floor rather than at it. Average weight loss in the trial cited on the page is 14.9%. The medication is the branded, FDA-approved product rather than a compounded copy. Molecule: semaglutide.

What is good about Wegovy?
- +One flat $349 across every standard dose from 0.25 to 2.4 mg — no dose-escalation penalty and no refill deadline attached, unlike Lilly's conditional pricing on Zepbound.[1]
- +New patients pay $199 for the first two fills through 31 December 2026 — the first two of the four titration months.[2]
- +Approved for cardiovascular risk reduction, not only weight — the strongest insurance-coverage argument in the category.[1]
- +The longest real-world safety record of any modern weight-loss GLP-1.[1]
- +Commercially insured, the savings card can bring it to $25 a month, capped at $100 of savings per month.[2]
What are the downsides of Wegovy?
- −$1,349 monthly list price without insurance or the self-pay program.[1]
- −Weaker than tirzepatide where the two were tested head to head: in SURMOUNT-5, weight fell 13.7% on semaglutide against 20.2% on tirzepatide over 72 weeks.[6]
- −A month is one box of four weekly pens, so a year takes 13 boxes, not 12: $4,537 at $349 rather than the $4,188 that 'per month' implies.[1]
- −The new high dose, Wegovy HD 7.2 mg, sits outside the flat price at $399.[1]
- −Weekly injection, refrigerated storage, GI side effects during titration, and weight regain after stopping is the documented norm.[3]
How this score is built
- 2.5/3.5Evidence — 14.9% mean weight loss, the intention-to-treat result on the FDA label
- 3.0/3Approval — FDA approved for cardiovascular risk reduction, chronic weight management from age 12, and noncirrhotic MASH with moderate to advanced liver fibrosis at stages F2 to F3 — the last under accelerated approval, which the label says may be contingent on a confirmatory trial.
- 0.9/2.5Cost — $349/mo at maintenance
- 0.5/1Caveats — −0.5: boxed warning −0.5
Clinical evidence

What Wegovy costs with insurance
There is no single figure, and any page giving one is describing a plan that is not necessarily yours. What decides it is below, and every part of it is checkable.
- Wegovy savings card: As little as $25 a month Who it is for: Commercial insurance that covers Wegovy. Government beneficiaries excluded. The catch: Savings are capped at $100 a month, so it only reaches $25 if your plan already covers most of the cost.
- Medicare GLP-1 Bridge: $50 a month. Who it is for: Part D beneficiaries 18 or older whose prescriber submits a prior authorization attesting the drug treats obesity, and who have a BMI of 35 or higher, a BMI of 30 or higher with diastolic heart failure, uncontrolled hypertension or stage 3a chronic kidney disease, or a BMI of 27 or higher with prediabetes, a previous heart attack or stroke, or symptomatic peripheral artery disease. Not for anyone with type 2 diabetes, moderate-to-severe sleep apnea or fatty liver disease, or whose Part D plan already covers a GLP-1. Covers all formulations of Foundayo and Wegovy, and the KwikPen formulation of Zepbound only — not the single-dose Zepbound vials or pens. The catch: The $50 does not count toward your Part D deductible or the out-of-pocket maximum, because the demonstration sits outside Part D coverage rather than inside it. It is also time-limited: a demonstration under Section 402 authority, not a benefit. The full terms, and every drug it covers.
- It is closed to you if a government program pays for your care. Manufacturer copay cards exclude Medicare, Medicaid, TRICARE and VA beneficiaries by their own terms — a federal anti-kickback rule rather than a plan decision, so there is nothing to appeal. Medicare's own route is the separate one above, and it is not a manufacturer card. Which coupons you can actually use.
- The indication on the prescription. A plan that excludes weight-loss drugs excludes the use, not the molecule — and Wegovy is separately approved for cardiovascular risk reduction, which that exclusion does not reach. What each drug is approved to treat.
- If the answer is no. The cash price is what remains, and it is on this page. Every verified route to the same drug is priced with the date each was read.
