Ozempic (semaglutide, diabetes label) dosage schedule
One injection a week. The FDA label below escalates it on a schedule, and the maintenance dose is 0.5 mg to 2 mg once weekly, reached no earlier than week 5.
| When | Dose | What the label says about it |
|---|---|---|
| 1–4 | 0.25 mg | Initiation, for 4 weeks, to reduce stomach side effects. The maintenance doses for glycemic control are 0.5 mg, 1 mg and 2 mg. |
| 5–8 | 0.5 mg | — |
| 9–12 | 1 mg | Also the maintenance dose for people with type 2 diabetes and chronic kidney disease, reached after at least 4 weeks at 0.5 mg. |
| 13 and on | 2 mg | Only if more glycemic control is needed. |
Maximum dose: 2 mg once weekly. The label sets nothing above it.
Does the price change as the dose goes up?
The first rise follows the fill count, not the dose: NovoCare, Novo's own pharmacy, charges a patient new to the Ozempic Savings Offer $199 for each of the first two fills at 0.25 mg or 0.5 mg, through 31 December 2026, then $349 a month at every dose up to 1 mg and $499 at 2 mg. Two milligrams is Ozempic's ceiling; Wegovy, the same molecule, goes to 2.4 mg, and to 7.2 mg (Wegovy HD) for weight reduction in adults. Ozempic is approved for type 2 diabetes, not weight, so prescribing it for weight loss is off-label, which is where the insurance argument starts.
halfrx records Ozempic (semaglutide, diabetes label) at $349/mo at the maintenance dose, verified 2026-09-22.
What happens if I miss a dose?
Take a missed dose within 5 days; after that skip it.

Can you stay on a lower dose to save money?
The starting dose is cheaper, which is why the question gets asked. What it saves, what the label calls that dose, and whether the cheap rate belongs to the dose or only to the first fills.
Questions
- How long does it take to reach the maintenance dose of Ozempic?
- At the fastest the label allows, week 5. Each step has to be held for its full interval before the next one, and a step can be repeated if the current dose is not tolerated — so the real answer is usually longer than the chart, never shorter.
Every dose and interval on this page is from FDA prescribing information for Ozempic (semaglutide) injection, diabetes label , the version published 2026-06-10. Nothing here is a recommendation: the dose you take is the one your prescriber sets, and the schedule is often slowed deliberately for tolerability.
