Does North Dakota Medicaid cover GLP-1s for weight loss?
North Dakota Medicaid covers a weight-loss drug but not a GLP-1 on that indication. Coverage for type 2 diabetes, for Wegovy's cardiovascular indication and for Zepbound in moderate to severe sleep apnea is required of every state programme regardless, which is why the practical question at a denial is rarely "does my state cover it" but "which indication am I being prescribed this under". The coverage status comes from North Dakota Medicaid Preferred Drug List and Prior Authorization Criteria, version 2026.6. This state's own criteria document has not been read yet, so no threshold, step-therapy rule or renewal term is printed — the status is known, the conditions attached to it are not. The exclusion is permitted rather than required: section 1927(d)(2) of the Social Security Act lets a state Medicaid programme leave out drugs used for weight loss, which is why this is a state decision and not a fact about the drug.
Phentermine, bupropion, naltrexone and topiramate are covered for obesity with no prior authorisation, on a claim that carries the obesity diagnosis code. No GLP-1 is covered for weight; Wegovy is approved only to reduce cardiovascular risk, for members aged 55 to 74 without diabetes, with a BMI of 27 to 35 and established cardiovascular disease, and only after Ozempic at 2 mg has been tolerated.
Whatever North Dakota decides about obesity, federal law still makes it cover a GLP-1 prescribed for type 2 diabetes, Wegovy for cardiovascular risk reduction and Zepbound for obstructive sleep apnea — the exclusion is written against the use, not the drug.
