Does California Medicaid cover GLP-1s for weight loss?
California Medicaid does not cover GLP-1s prescribed for weight loss. Coverage for type 2 diabetes, for Wegovy's cardiovascular indication and for Zepbound in moderate to severe sleep apnea is required of every state program regardless. So the practical question at a denial is rarely "does my state cover it" but "which indication am I being prescribed this under". The status is read from the state's own document, Medi-Cal Rx: Changes to GLP-1 Drug Coverage, effective 1 January 2026, rather than from a national summary. The exclusion is permitted rather than required. Section 1927(d)(2) of the Social Security Act lets a state Medicaid program leave out drugs used for weight loss, so this is a state decision, not a fact about the drug. Where the answer is no, the cheapest verified cash route on this site is $129 a month (compounded, not FDA approved; FDA warning letter). A no here is about the obesity indication alone.
What California changed, and when
- 2026-01-01 — GLP-1s for weight loss dropped entirely; every existing approval expired on 31 December 2025 with no continuing care the document that says so
Medi-Cal Rx stopped covering GLP-1s for weight loss on 1 January 2026, whatever the BMI. Wegovy, Zepbound and Saxenda approvals ended on 31 December 2025 and there is no continuing care. Wegovy stays covered for MASH and, with prior authorization, for cardiovascular disease; Zepbound for obstructive sleep apnea; the diabetes GLP-1s on a diabetes diagnosis code. Under-21s keep access through EPSDT where a request is approved, and a denial carries the right to a State Hearing.
Whatever California decides about obesity, federal law still makes it cover a GLP-1 prescribed for type 2 diabetes, Wegovy for cardiovascular risk reduction and Zepbound for obstructive sleep apnea — the exclusion is written against the use, not the drug.
