GLP-1s and menopause: what has been measured
No FDA label mentions menopause, and none reports results by menopausal status. What exists is one small study, and it points at hormone therapy.
Do GLP-1 drugs work for weight loss after menopause?
There is no label figure for this group: no label reports results by menopausal status. The most specific study is a Mayo Clinic cohort of 106 postmenopausal women on semaglutide. Those also taking menopausal hormone therapy lost 16% of body weight at 12 months, against 12% for those who were not, and the gap held after adjusting for differences between the groups.
It is a retrospective study of 16 women on hormone therapy and 90 without, so it shows an association, not that hormone therapy causes the difference; its authors call for larger studies. None of the labels mentions menopause or menopausal hormone therapy. For women in perimenopause still relying on the pill, the tirzepatide contraception warning applies at any age.

Retrospective cohort of postmenopausal women treated with semaglutide for at least 3 months: 16 on hormone therapy, 90 not. Weight loss at 3, 6, 9 and 12 months was 7%, 13%, 15% and 16% with hormone therapy against 5%, 9%, 10% and 12% without, and more women on hormone therapy reached 5% and 10% loss. The association held after adjustment; both groups' cardiometabolic markers improved.
GLP-1s and menopause, answered
- Should I start hormone therapy to lose more weight on a GLP-1?
- The study that found the difference was not designed to answer that: it compared women already on hormone therapy with women who were not. Hormone therapy has its own indications and risks, and the decision rests on those.
- Does Zepbound affect the pill during perimenopause?
- The same way at any age: the tirzepatide labels advise switching to a non-oral method or adding a barrier method for 4 weeks after starting and after each dose increase.
- Can GLP-1s treat hot flashes?
- No label mentions menopausal symptoms, and no randomised trial found here tested it.
