GLP-1s and inflammation: the marker that falls, and what the labels claim

Inflammation is the benefit most often credited to these drugs and one no label states. What has been measured is C-reactive protein, and it falls.

Do GLP-1 drugs reduce inflammation?

The marker the trials measure says yes. A 2026 meta-analysis of nine randomised trials of injected semaglutide, 6,239 adults with overweight or obesity and no diabetes, found C-reactive protein fell 40.9% more than on placebo, alongside 12.0% more weight loss. CRP is a general blood marker of inflammation, not a measure of any one disease.

No FDA label claims an anti-inflammatory effect, and the trials cannot separate how much of the fall comes from the weight lost rather than the drug. The nearest thing on a label is Wegovy's cardiovascular indication: it is approved to reduce the risk of cardiovascular death, heart attack and stroke in adults with established heart disease and overweight or obesity. That is an outcome, not a mechanism.

What the trials measured

Nine placebo-controlled trials of subcutaneous semaglutide in 6,239 adults with overweight or obesity without diabetes: body weight −12.04% (95% CI −13.08 to −11.00), waist −9.36 cm, C-reactive protein −40.90% (−46.37 to −35.42) against placebo. Serious adverse events did not differ (risk ratio 1.12, 0.76 to 1.65).

Which GLP-1 is best for inflammation?
None is approved or labelled for it, and no trial found here compared them on inflammation. The pooled CRP figure is for semaglutide injections; the 2026 analysis did not include tirzepatide.
Is lower CRP the reason for Wegovy's heart benefit?
That has not been established. Wegovy's cardiovascular indication rests on fewer cardiovascular deaths, heart attacks and strokes, not on CRP. Lower inflammation is one proposed explanation, alongside weight, blood pressure and blood lipids.
Can a GLP-1 treat an inflammatory disease?
No label on this site includes one, and a falling CRP is not evidence that any particular inflammatory condition improves. Trials in specific diseases would have to show that.
